RN Case Manager (Hospitalist)
**Summary:**

Assumes responsibility and accountability for a group of patients for a designated time frame and provides care to these patients through the therapeutic use of self, the nursing process, the environment/instrumentation, and other health care team members.

**Essential Duties and Responsibilities** include the following. Other duties may be assigned.





+ Collaborate with Hospitalist regarding patient assignment.

+ Actively participate in department performance excellence huddle, followed by multidisciplinary huddle on the patient floors.

+ Formulates plan of care with Hospitalist during patient rounds, which includes real-time physician orders.

+ Provides an appropriate care plan for each of the subspecialty areas, i.e. surgical, medical, progressive care units, and cardiology.

+ Evaluates the efficiency and efficacy of orders, consults, and treatment plans with ongoing review for quality patient care.

+ Communicates and executes inpatient plan of care with the interdisciplinary team and patient.

+ Facilitates an efficient disposition process in conjunction with the physician.

+ Identifies barriers to achieving goals in the plan of care, and uses critical thinking skills to resolve.

+ Professionally presents self to patient / family as a representative of the Hospitalist team.

+ Educate and facilitate critical thinking with primary nursing regarding acute patient medical management.

+ Assures completion of all discharge paperwork including, but not limited to, discharge summary, FMLA, return to work statements, prior authorizations and attending physician forms.

+ Coordinates post-discharge follow-up care with accepting facility, primary care or sub-specialty office. Serves as a safety net during patient hand off to primary care physician.

+ Participates in hospital committees and special projects to improve team or hospital performance.



**Education/Experience:**

Graduate of an accredited school of nursing, BSN or MSN required. Meets educational preparation or other qualifications within specified timeframes as determined/outlined by specific department policy.


Five years of experience in hospital-based medicine preferred. Knowledge of professional nursing theory and practice to give and evaluate patient care. Knowledge of organizational policies, regulations, and procedures to administer patient care. Knowledge of common safety hazards and precautions to establish a safe work environment.


**Certificates and Licenses:**

Current RN licensure in the State of Kansas.

BLS certification required within 90 days of hire.
